New Patient Registrationregister_now

Currently all three practices in Bonnyrigg Health Centre are operating on a ‘restricted but open’ basis. This means that for the time being we will only register certain categories of patients, as follows:

 New babies of existing patients

Immediate family members of existing patients who

become resident within the registered patient’s address

 

Temporary residents

Assignments / allocations

 

Therefore, in order to register with a practice in the area you will be required to call Practitioner Services on the number below where they will allocate you to a practice by letter. This is to ensure a fair allocation of new patients across all three Bonnyrigg practices.

 

Please bring this letter with you when you attend your allocated practice for the registration forms.

No patient moving into the area will be left without a GP.

Once you have been allocated, you will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All adults dated 16 and over will need to provide proof of identification and residency.

You need to provide one of the following:

valid passport or valid photo ID driving licence or NHS medical card or birth cerificate

and one of the following:

a utility bill dated within one month of registration or a council tax bill / rent later dated within one month of registration or a bank / credit card statement dated within one month of registration.

Children under 16 need only provide a valid passport or NHS medical care or birth certificate.

If you don't have any of the above, please speak to ta receptionist who will advise you on what we could accept instead

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration.
