New Patient Registration
Dalhousie Medical Practice is open to new patient who live within our practice area. To register, please contact us via email at firstname.lastname@example.org to request a copy of the registration form or via phone at 0131-3703999.
Due to the COVID restrictions, we accept new registrations via email. Please complete the registration form and return to the above email address. All adults aged 16 and over will need to provide proof of identify and residency, therefore you need to attach a copy of one of the following with your registration form:
- valid passport
- valid photo ID driving licence
- NHS medical card
- birth certificate.
Plus, one of the following:
- utility bill dated within one month of registration
- council tax bill
- rent letter dated within one month of registration
- bank or credit card statement dated within one month of registration.
Children under 16 need only provide a valid passport, NHS medical card or birth certificate.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that an accurate note of any repeat medication you may be taking.
Medical treatment is available from date of registration.
