New Patient Registration Changes

Due to the unprecedented demands on our GP services, we only accept registrations from patients who are not registered with a local GP surgery or who are new to the Bonnyrigg area. 

Please be aware that it can take up to two weeks for your registration form and questionnaire to be authorised. We do not operate a waiting list for those wishing to join our list.

To register, please email us a completed registration form and questionnaire to prescriptions.s77055@nhslothian.scot.nhs.uk The forms are available here:

  1. The Patient Registration form from NHS Inform here and 
  2. Adult Questionnaire  or Under 16 Questionnaire 

If you can't download these, you can request a copy by emailing prescriptions.s77055@nhslothian.scot.nhs.uk or phoning 0131 3703999.

It is not mandatory to submit proof of identity and residency but our practice policy is that we kindly ask you to provide one proof of identity and residency (an up to date driving licence can show both) for administrative reasons during your registration. This can include but is not limited to:

  • valid passport
  • valid photo ID driving licence
  • NHS medical card
  • birth certificate
  • utility bill dated within one month of registration
  • council tax bill
  • rent letter dated within one month of registration
  • bank or credit card statement dated within one month of registration.

Children under 16 need only provide a valid passport, NHS medical card or birth certificate.

Medical treatment is available from date of registration.
